Support Coordinator

Beyond Care is looking to recruit a NDIS Support Coordinator to assist clients who require services under their NDIS plans. As a Support Coordinator, you will be responsible for assisting National Disability Insurance Scheme (NDIS) participants in activating, interpreting and implementing their NDIS Plans, with a strong focus on seeing participants move towards their NDIS goals. You will work closely with NDIS participants and their families/guardians to connect with individual services and supports outlined in their NDIS Plan, ensuring they get the most out of their funding and achieve desired outcomes.

Responsibilities include:

  • Work in consultation with clients and their families to find the services and supports that match their needs and funding
  • Research, coordinate and manage a range of supports to suit individual needs across
  • Respond to new support co-ordination enquiries for new participants.
  • Meeting their goals and exercising choice in obtaining supports that assist them in pursuing their own lifestyles.
  • Provide support to participants to increase their independence, social participation, and economic participation so they can take more control of their lives and to better manage complex challenges of day to day living
  • Assist clients, families and carers to connect with services both mainstream and within disabilities
  • Develop, monitor, review and report on your client’s NDIS plans and share progress with key stakeholders
  • Work within the funding allocated and regularly monitor and track billable coordination hours
  • Building and maintaining relationships with our clients, their families, staff, peers, other services and stakeholders
  • Respond to new support co-ordination enquiries for new participants.

About You

Beyond Care is looking for a candidate with a genuine desire to support individuals with disabilities lead exceptional lives. This is an important role within Beyond Care, and we are looking for candidate with the following:
  • Strong knowledge of the NDIA, NDIS plans and funding categories
  • Qualifications in Social Work, Allied Health, Case Management or equivalent experience is desirable
  • Previous, sound experience working within the disability industry
  • Strong administration skills
  • Ability to manage and deliver to multiple deadlines
  • Current Australian drivers licence and car
  • Clear, effective communication skills - both written and verbal
  • Highly computer literate
  • A clean, unrestricted drivers’ licence
