From a “temporary place” to “home” – Simon’s journey at Evan Street.

When Simon first moved into the Evan Street Supported Independent Living (SIL) home in February 2026, the transition presented several challenges. Like many people adjusting to a new living environment, Simon required support to establish routines, settle into his new surroundings and develop meaningful connections within the home.

Over the past year, Simon has made remarkable progress in achieving his transition goals and building a stable, fulfilling life at Evan Street.

A key achievement has been the implementation of tailored support plans, routines and individualised strategies designed to promote Simon’s wellbeing, independence and quality of life. These supports have enabled staff to provide consistent, person-centred assistance while encouraging Simon to actively participate in daily life and make choices that reflect his preferences and goals.

Simon has successfully achieved all key goals identified within his transition plan. Staff worked closely with Simon, his family and relevant support professionals to establish routines that support his independence, community participation and overall wellbeing. He now takes part in regular community outings, enjoys recreational and leisure activities and maintains strong connections with his family.

Connecting with the Evan Street Community

One of Simon’s most significant achievements has been his successful integration into the Evan Street household community. Simon enjoys spending time with the other residents, regularly joining them to watch television, socialise and take part in weekend barbeques. These shared experiences have fostered positive relationships, reduced social isolation and strengthened his sense of belonging within the home.

Staff have observed a noticeable increase in Simon’s confidence, engagement and enjoyment of daily life. He particularly looks forward to community outings and recreational activities, which have become an important part of his weekly routine.

The success of Simon’s transition is perhaps best reflected in the language he now uses when talking about Evan Street. During his first few months, Simon often referred to the residence as a “temporary place.” Today, he proudly refers to Evan Street as “home,” demonstrating a strong sense of security, stability and connection to his living environment.

Simon continues to benefit from the collaborative support of his family, support staff and the professionals involved in his care. Their ongoing partnership ensures he receives the assistance and opportunities he needs to keep working towards his personal goals and enjoy a meaningful lifestyle.

Outcome

Simon has successfully transitioned into his new home, achieved all identified transition plan goals, established strong social connections, increased his community participation and developed a genuine sense of belonging. Evan Street is no longer a temporary place for Simon; it is now his home.

While his transition goals have been achieved, staff remain committed to supporting his ongoing NDIS goals – promoting wellbeing, community participation, social engagement and strong family connections – to ensure his progress and quality of life are maintained and enhanced into the future.
